• Stamp paper for Rs. 200/- 
• Policy number
• Name of policy number
• Date of Commencement of the policy
• Sum assured
• Request letter
• Photo ID
• Address proof.

After collecting all details, get the format typed in the stamp paper. Along with all relevant documents submit at the particular branch.
